Welcome to Jack Sears House

At Jack Sears House, we offer short‑term, person‑centred reablement for people who need a bit of extra support after a hospital stay or a period of ill health. Our aim is simple: to help people rebuild confidence, regain independence, and feel ready to return home. We work closely with the NHS, and our on‑site clinical and therapy teams provide focused, goal‑based support that’s shaped around each person’s needs. Most people stay with us for up to 21 days, and during that time we work together to make sure they’re safe and prepared for life back at home.

A big part of our approach is “doing with” rather than “doing for.” We encourage people to be as involved as possible in their own recovery, while our team offers guidance, reassurance, and practical help along the way. That might mean support with everyday tasks, mobility, personal care, or rehabilitation exercises — always delivered in a way that builds independence rather than creating reliance.

For many people, Jack Sears House is the step between leaving hospital and going home. We support those who are medically stable and no longer need to stay in an acute hospital, but who aren’t quite ready to manage safely at home. This might be because of reduced mobility, difficulty with daily activities like preparing meals or washing and dressing, or simply needing more time to regain strength. Once someone is medically stable, they need to leave the hospital, and our NHS intermediate care service provides the space and support needed for rehabilitation and recovery.

To help make the move home as smooth as possible, we sometimes carry out “back‑to‑home” visits. These visits help us understand someone’s home environment and make sure everything is in place for a safe return. That might include arranging equipment or organising extra care to support them once they’re home.
