What will you need during your visit?
For your stay, Jack Sears House provides storage in your bedroom for personal items, with bedlinen and towels supplied and laundering services available. You should bring nightwear, day clothes, toiletries, spectacles, and hearing aids with spare batteries. Also, consider any religious items, mobility aids, reading materials, and electrical devices (which require staff checks).
Valuables are not recommended, but a lockable drawer and safe options are provided for secure storage upon request.
Each room has its own lockable medication cabinet. We will ask for a deposit for the use of the keys. If you bring any personal electronic items these will need to be PAT tested
Your Care Plan
Upon arrival at Jack Sears House Rehabilitation Centre, a personalised care plan is promptly devised in collaboration with you, the unit lead and the intermediate care team.
This plan, tailored to your preferences and goals, addresses assessed care needs, potential risks, and necessary interventions. It outlines interactions with healthcare professionals, prescribed treatments, and dietary considerations. Your plan is securely stored electronically but you'll also receive a paper copy if requested for reference.
